Support at Home - Aged Care in Perth

 The Australian Government is reforming in-home aged care through Support at Home. It expands the previous Home Care Package program to 8 levels of care, introduces the Restorative Care Pathway and the End-of-Life Pathway and improves access to products, services, equipment and home modifications. It also reduces funding wait times for ongoing and short-term care services.

What is Support at Home?

If you are eligible, you can start receiving home care services as soon as funding becomes available and you have signed a service agreement. Your service provider can help you decide what mix of services best meet your assessed needs and budget. Funding wait times vary by service type and priority category.

Support at Home Perth is the Australian Government program that replaces the previous Home Care Packages Program. It expands home care to 8 levels of support and introduces three short-term pathways — the Restorative Care Pathway, the End-of-Life Pathway and the Assistive Technology and Home Modification Scheme.

The program offers a simpler way to manage your aged care. It has shorter wait times for home care, more choice of service providers and a better way to use your home care budget.

Funding

The Australian Government has reformated in-home aged care funding and replaced the Home Care Packages Program and Short-term Restorative Care Programme with Support at Home. It expands home support to 8 levels of care, including a new Restorative Care Pathway, End-of-Life Pathway and Assistive Technology and Home Modification scheme. Each of these levels has a different level of funding depending on your assessed needs. When your funding arrives, you can start using approved services from the provider of your choice. You may be required to pay a contribution towards the cost of the services based on your income and assets.

Service providers

The Australian government subsidises the cost of home care services that can help you live independently at home. If you are approved for ongoing Support at Home, you can choose your own service provider from a list of organisations that offer a range of services. These include allied health services, like in-home physiotherapy; nursing care; and other products, equipment and home modifications. The new Support at Home program replaces the previous Home Care Packages Program and Short-term Restorative Care Programme. It expands home care funding levels to 8 different classifications with funding for care management and includes 3 short-term pathways: the Restorative Care Pathway, the End of Life Pathway, and the Assistive Technology and Home Modification scheme. These can lower the need for ongoing care.
