Support at Home For Aged Care in Perth

 Support at Home Perth is the Australian government’s new coordinated in-home care program. It replaces the Home Care Package Program and short-term restorative care programs. It offers a range of services with 8 funding classifications, 3 short-term pathways and a defined service list.

A personalised home care assessment will check if you are eligible and how much funding you can receive. You’ll then be allocated a budget and can choose from a wide range of providers.

From 1 November 2025, the Australian Government will be introducing a new in-home aged-care program called Support at Home. This will replace the Home Care Packages Program and the Short-term Restorative Care Program.

If you already have a HCP, your ongoing funding will transfer to the new system, but your level of care and budget will remain the same. If you’re on the Priority System or waiting for a HCP, you will be offered interim funding.

When you’re ready to start getting care, you can find providers who offer the services you were approved for during your assessment through My Aged Care. Then you can give them your referral code from your Notice of Decision letter and start developing a care plan.

Our services

With Support at Home, Australia’s government-funded in-home care is streamlined into one program. It replaces the Home Care Packages (HCP) and Short-Term Restorative Care (STRC) programs.

You can use your Support at Home classification budget for a range of home care services, including ongoing support and personal care, nursing, domestic assistance, meals, transport, assistive technology, and minor home modifications. The new system also includes 3 short-term pathways that provide short-term support to help you regain function, reduce your dependency on ongoing services and stay at home longer.

The Australian Government’s new Single Assessment Tool and Integrated Care Plan will be used to assess your needs and goals, and determine which home care services you can receive. You will receive a Notice of Decision and a Support Plan with your approved service mix, which can be changed at any time if your circumstances change. You can choose a provider from an approved list of providers, like Southcare, who will work with you to create a personalised care package.

The program requires a minimum contribution from you, but the Government subsidises the cost of your clinical and everyday living services. You will be asked to undergo an income and assets assessment by Services Australia, as this determines how much you will need to contribute.

Once you have been approved, you will need to choose a provider and discuss with them the mix of services that best meets your assessed needs and budget. This will be outlined in your Notice of Decision letter. You can also request a reassessment if your circumstances change.
